The Well Communities CIC (The Well) is led by people in recovery from substance misuse. We are working in partnership with CGL (Change, Grow, Live) to develop and deliver an innovative Behaviour Change Service to the Communities of North & Central Lancashire.
You will be someone who shares our belief in the inherent capacity and potential of people to bring about positive change; for themselves and the communities in which they live. In this role you will use your experience and skills to engage with an identified cohort of service users with multiple, complex and additional needs. These service users are at an elevated risk of disengaging from treatment with multiple barriers preventing them engaging effectively with treatment services. The role will increase access to relevant health and social care provisions for this cohort.
This is an assertive role where you will pro-actively engage with service users to support them accessing community services, maintaining appointments with the substance misuse team, pharmacies, primary care, specialist services and support integration into Behaviour Change Activities to build their Recovery Capital and access their community support whilst being sensitive to their needs and wishes.
The post holder will utilise their skills and experience and deliver Harm Reduction Interventions whilst maintaining key relationships and engaging with wider teams to develop partnerships and pathways to relevant agencies delivering and contributing to specific outreach activity.
